Recording Clinical Frailty Scale scores for individuals

SystmOne

In SystmOne Clinical Frailty Scale scores as recorded as part of holistic assessments.

Ardens Practices:

  1. Access the template through the auto consultation button.
  2. Go to Ardens Conditions frailty and end of life and then select the Frailty template.
  3. Once in the template, go to the Diagnosis tab. The option to add a frailty status (SNOMED code) is on here as well as a button to launch the Edmonton frail scale and Rockwood Scale tools.

F12 practices and Community (Citycare):

  1. Access template through F12 data entry dashboard.
  2. Choose the Clinical Frailty scale F12 template. Both the scores and the SNOMED codes are built into this.
