The partners of The Forest Practice in Hyson Green (NG7 5HY) have taken the decision to serve notice on their GP contract. As a result, the last patient appointments will take place at Forest Practice on Monday 30 September 2024.
There is capacity for more patients at other nearby practices, so we have allocated patients currently registered at Forest Practice to a new practice near to where they live to ensure they continue to have good access to GP services in their local area.
We have now written to all registered patients outlining details of the GP practice they have been allocated to and how they can access GP services with effect from 1 October 2024. Letters have also been sent to patients registered with Forest Practice who live outside Nottingham and Nottinghamshire with details about what they need to do to register with a practice in their area.
In the meantime, patients do not need to take any action and can continue to contact The Forest Practice for their healthcare needs, up to and including 30 September 2024.
Patients have the opportunity to register with an alternative practice if they wish, provided it covers the area they live in. Patients are requested not to register at a different practice until the transfer process has been completed on 1 October 2024.
