Older and Wiser Guide
The Older and Wiser Guide is a professionally developed resource designed to support older adults in South Nottinghamshire to live independently, stay safe, and maintain wellbeing. It was produced collaboratively by the South Nottinghamshire Integrated Care Partnership (ICP), with input from local authorities, NHS partners, and voluntary sector organisations.
This guide brings together key information on local services, community support, and practical advice relevant to ageing well. It covers areas such as falls prevention, home adaptations, social engagement, and managing long-term conditions. It is intended to assist professionals across health, social care, and the voluntary sector in signposting older people and their carers to appropriate services in Broxtowe, Gedling, Rushcliffe, and surrounding areas.

Falls & Physical Activity Community of Practice
Active Notts launched the Falls and Physical Activity Community of Practice in 2021 to facilitate meaningful conversations about re-conditioning and falls prevention at every level (within community, clinical and care settings).
The community holds seasonal events and promotes an ethos of shared ownership to continue to share knowledge and ideas on how we can tackle the huge problem of deconditioning and create a system pivot shift towards prevention which will support our population in reconditioning in order to stay strong and live happier, healthier, independent and more active lives.
