This section has been created to support health and social care professionals across Nottingham and Nottinghamshire in their work with older adults who are experiencing or at risk of frailty, falls, and bone health deterioration.

Its purpose is to:

  • Signpost to training, community support programmes, and the latest local and national guidance
  • Provide guidance and training on the use of the Clinical Frailty Score (CFS) and how to record this across different systems
  • Promote early identification and proactive management of frailty and fall risk through evidence-based best practice
