Dynamic Support Register
The Dynamic Support Register (DSR) is a vital national tool designed to support the needs of people with a primary diagnosis of learning disability and/or autism. It helps identify those who may be at risk of crisis and ensures a coordinated response to meet their needs.
Referral criteria
The referral criteria to the DSR includes people:
- Who are at risk of mental health hospitalisation/or being considered
- Who place themselves or others at serious and/or significant risk of harm
- Whose risk, mental health and/or lack of support is impinging upon community placement or tenancy, residential special school or care and support placement is at risk of irretrievable breakdown and where this would pose a significant risk to the safety and wellbeing of the person and/or others
- Who are presenting to the police
- Who are presenting to urgent and emergency care services due to mental health.
- Who have recently been discharged from a mental health hospital
- Who have been supported by a mental health crisis team in the last year
- Who have one or more factors that mean they are at increased risk of admission.
This is not an exhaustive list; and all the above criteria may therefore require a multi-professional response, be monitored and/or prioritised for additional support in response to a person’s increasing needs or risks in which an enhanced support meeting such as the DSR is required to monitor their care and treatment as a team.
Why the DSR is important
- Proactive care: the DSR helps professionals respond quickly when a person’s needs change, preventing escalation into more serious issues.
- Crisis prevention: by identifying those at risk early, the DSR helps reduce the likelihood of emergencies or mental health hospital admissions.
- Holistic support: the register ensures that all aspects of a person’s life, school, home, education, and health are considered in their care plan.
How to access the DSR
If you believe your child or an adult could benefit from being on the Dynamic Support Register, speak with your GP, school Special Educational Needs Co-ordinator (SENCO), or a healthcare professional. They will assess their needs and, if appropriate, refer them to be included on the register.
You can make a self-referral to the DSR but you will need help from a health or social care representative.
Being part of the DRS aims to ensure that a person’s needs are closely monitored and that they receive the support they deserve. It’s one more way we work together to create a safer, more supportive environment for people with complex needs.
How information from the DSR will be used
You will be told why your confidential information is shared to the DSR and how it will be used. You will be told:
- What information is being shared,
- Who your information will be shared with
- How your information will be used.
Your information will be recorded and shared for the purposes of the DSR, and this is done so to support safe and effective care.
Information discussed at the DSR is strictly confidential and will not be disclosed to third parties who have not signed up to the DSR Information Sharing Agreement without consent.

Key Working Service
Autism and Learning Disability Key Workers ensure children and young people aged 0-25 who are on the Dynamic Support Register, and their families/carers receive timely access to the right personalised care and support, reducing the risk of hospital admission.
