We use your data to employ data-driven approaches to enhance the health and wellbeing of our community. This initiative is called Population Health Management and integrates local and national data from:
- GP Practices
- Social Care services
- Mental Health services
- Hospitals
- Community care
- Out-of-hours care
By joining the data dots across multiple organisations, we are able to develop a comprehensive understanding that informs local health initiatives, facilitates illness prevention, and ensures prudent allocation of public resources.
Safeguarding Your Data
Any information we hold is protected by the highest levels of security. To help improve the health of people in our area we analyse information from health and care records and share data with other public sector organisations. However, this will never include your name or anything that could identify you. All participating organisations must comply with stringent data protection legislation.
The use of your data has been formally reviewed by the Confidentiality Advisory Group (CAG), an independent panel of lay members and professionals who provide expert guidance on the use of confidential patient information without explicit consent. CAG recommended approval of our application, which was subsequently endorsed by the Secretary of State for Health.
It is important to us that you have control over the use of data about you and how your data is stored and managed.
